Homebased | North East | Lifeways
At Lifeways, we're looking for an experienced social care assessment professional who understands complex needs and is ready to take the next step into leadership.
You could be a Senior Assessment Practitioner, Social Worker, Occupational Therapist, Clinical Assessor, Care Manager or similar professional with strong experience assessing adults with complex needs.
Around 50% of the role will remain hands-on, managing your own referrals and leading complex assessments. The rest will focus on supporting and coaching Assessment Practitioners, helping improve assessment quality and developing your leadership experience.
Manage your own caseload of referrals and lead complex assessments.
Assess needs, risks, compatibility and suitability of referrals.
Make confident decisions about whether support can be safely and sustainably delivered.
Support and coach Assessment Practitioners.
Work closely with Business Development, Operations, Quality and PBS teams.
Build relationships with commissioners, local authorities, NHS teams, social workers and other referrers.
Use your sector knowledge and relationships to help progress suitable referrals and opportunities.
Identify risks and barriers early and support effective mobilisation.
Strong social care assessment experience
You understand complex needs and are confident making professional judgements around risk, suitability and deliverability.
Leadership potential
You may already have leadership or supervisory experience, or you may be ready to take your first step into a leadership role. You enjoy supporting others and sharing your expertise.
Strong sector relationships
You understand the importance of building trust with commissioners, referrers and other professionals and can use those relationships to help move the right opportunities forward.
Commercial awareness
You don't need a Business Development background, but you'll understand how good assessment, strong relationships and sustainable growth work together.
This is a great opportunity to step into leadership without stepping away from assessment.
You'll continue doing the work you enjoy while developing your leadership skills and having a real influence on the referrals we accept, the services we develop and the people we support.
If you're an experienced social care assessor looking for your next step, we'd love to hear from you.
